Raymond Avenue Veterinary Hospital offers our patient form online so you can complete it at your own convenience. This allows us to record your pet and owner information so that we can effectively and efficiently provide the service you deserve!

Payment Options
- Payment is expected when services are rendered. In order to focus on our patients' needs, customer service and minimizing costs, we do not bill.
- We accept credit cards (Visa, Mastercard, American Express), checks and cash. All cards must be signed by the owner of the card.
- When unexpected illness strikes a pet, unexpected expense strikes as well. The staff at Raymond Avenue Veterinary Hospital understands this and is able to make special arrangements through the CareCredit program. It takes about five minutes to complete an application and will allow you to break down your payment into three monthly installments. You can either apply at our hospital, where we will call in your application over the phone for immediate approval, or apply yourself online at www.CareCredit.com.
